Tava Guide includes a Referral Management Dashboard on the home page, along with a recent activity log, to help care navigation teams track member progress across their caseload.
The dashboard includes four widgets and can be filtered by care manager and timeframe.
Toggle to view engagement metrics from the last 2 weeks or all time.
Track metrics for the full team, or select individual care managers to filter the dashboard.
The dashboard shows four metrics:
- Members Added: total count of members added during the selected timeframe.
- Intake Completed: percent of added members who completed intake.
- Appointment Scheduled: percent of added members who have a first appointment on the schedule.
- Appointment Attended: percent of added members who have attended an appointment.
The dashboard also includes a live activity feed showing recent member activity, directly linking to their profile:
- Recently Added members: New members added, with timestamps and current intake status.
- Recent Activity: Any changes to member intake progress including intake completion, appointment scheduling, appointment attendance, and account logins.
Use the referral management dashboard to understand how members are progressing from referral to first session, track recent performance, and identify who may need follow-up.
