Minors aged 13-18 must have a Minor Consent Form signed by a parent or guardian before they can schedule a therapy session.
Adding a New Minor
To add a minor, follow these steps using the "Add Member" button, with a few extra steps. If the member's date of birth is detected to be under 18, you will be prompted to input a parent or guardian’s email for consent. Note that the parent and member cannot have the same email, they must be unique.
After clicking "Add Member," a consent form will be sent to the parent or guardian's email. You will not be able to add a provider to their care team until the consent has been signed. Once the consent form is complete, the member will receive an email to complete their intake, as outlined here.
Therapy for Minors Under 13
- Tava does not offer individual therapy for minors under age 13.
- However, they may attend family therapy sessions if a parent or guardian is present. In this case, a Minor Consent Form is not required.
